By filling out this form you can request access data for the Together App, either for yourself or for a represented person (parental authority/legal guardian).

Information to be considered for application

The data entered will be verified with those already in our systems. In case of a match, login credentials will be sent to the home address. If the data do not match, it will not be possible to proceed with the request. In that case, you will be contacted by EOC and will need to personally go to one of the EOC hospitals to update your master data and receive the sheet containing the access credentials.
