Types of requests

Request 1 - Specific documents
You can request one or more specific documents, such as an electrocardiogram or discharge letter. You must indicate precisely the desired information to facilitate identification of the documents.

Request 2 - Clinical and para-clinical elements of the medical record.
Includes a comprehensive compilation of key information that is most useful and relevant to understanding the course of care, such as discharge reports, outpatient visits, laboratory tests, radiological and other investigative reports (e.g., gastroscopy, colonoscopy, bronchoscopy, respiratory tests, cardiac stress test, electroencephalogram), surgical reports, final prescriptions, certificates, and prescriptions. This option provide a clear picture of the case without a surfeit of technical documents, which may be difficult for laypersons to interpret.

All documents for this type of request, are available automatically by downloading the "Together EOC" App. By downloading the App, documents up to 2012 are retroactively uploaded.

Request 3 - Complete medical and nursing documentation.
Includes all documents from the hospitalization, such as medical and nursing diaries, examination requests and consents. It provides a detailed view of the care received, but generates a high volume of technical documents that are often difficult for non-experts to interpret. For this reason, we recommend requesting this option only if it is strictly necessary to access all the details of the care pathway.

We point out that requests of this type, should the processing require significant administrative work, may be required to pay a financial contribution, in accordance with Article 24 of the LPDP and Article 19 of the OPDa. Such a contribution will never exceed 300 CHF.

Participation in the cost of issuing clinical documentation

Requests of type 1(Specific documents) and type 2(Clinical and para-clinical elements of the medical record) carry no cost.


For Type 3(Comprehensive Medical and Nursing Records) requests, should processing require significant administrative work, a financial contribution may be required, in accordance with Art. 24 of the Data Protection Act (DPA) and Art. 19 of the Data Protection Ordinance (DPO). Such a contribution will never exceed CHF 300.

Request for documentation of deceased patients

A request for the medical records of a deceased patient can only be processed after receiving a release from the professional secrecy to which all health professionals are subject. The authority in charge of releasing professional secrecy is the Cantonal Physician. Therefore, in these cases your request must be accompanied by a written and signed justification and, if available, the document (Family Status) attesting to the relationship with the patient.

We inform you as of now, that in case of refusal of release of professional secrecy by the Cantonal Physician, we will not be allowed to hand over the documentation to you.
